Important End-of-Life Documents
Beyond our familiarity with the “Last Will and Testament”, the document that tells family and friends what to do with our estate, there exist other documents that protect our wishes before we die, when we can’t speak for ourselves. This page is developed on the premise that you want to discover what they are and how to make sure they are accessible by your Health Agent, family members, doctors, hospitals and/or care facilities.
Those documents include, at a minimum:
Designation of Health Agent – Your health care agent is the person you choose to make medical decisions if you can’t.

Be sure to share your preferences with your health care agent.
Advanced Directive – Not required to be prepared by a lawyer, but can be.
